IMPORTANT NOTICE

This is a legal document. By signing it, you are agreeing to release legal rights, including the right to sue. Please read carefully before signing.

I, the undersigned, agree to participate in martial arts activities provided by the Kali Filipino Martial Arts Association, Inc. (hereinafter referred to as “the Association”), the umbrella organisation overseeing such training, under the following terms and conditions:

1. Acknowledgment of Risks:

I acknowledge that participating in martial arts activities, including training, sparring, self-defense drills, weapons practice, and physical conditioning, involves inherent risks, including but not limited to:

• Physical injury (including serious injury, disability, or death);

• Use of training weapons and physical contact;

• Risk of exposure to infectious diseases;

• Environmental hazards at training venues.

I understand that while all reasonable steps will be taken to minimise these risks, they cannot be eliminated entirely.

2. Medical Fitness:

I declare that I am physically and mentally fit to participate in martial arts activities and have not been advised otherwise by a qualified medical professional. I agree to notify the Association of any medical condition, injury, or disability that may affect my ability to participate safely.

3. Voluntary Participation:

I understand that my participation is voluntary and that I may withdraw at any time. I participate entirely at my own risk and accept full responsibility for my own actions.

4. Waiver and Release:

To the fullest extent permitted by law, I hereby release and discharge the Association, its instructors, officers, volunteers, employees, members, participants and agents from any and all liability, claims, demands, actions, or causes of action arising from any injury, loss, damage, or death that I may suffer as a result of participation in any martial arts activity conducted or sanctioned by the Association, whether arising from negligence or otherwise.

5. Indemnity:

I agree to indemnify and hold harmless the Association from any and all claims, liability, damage, or loss (including legal costs) arising from:

- My own actions during participation,

- Any breach of this agreement,

- Any injury caused to another person by me.

6. Insurance:

I acknowledge that any insurance held by the Association may not cover all possible injuries or losses. I accept that it is my responsibility to seek additional personal insurance if desired.

7. Photography and Media:

I give permission for photographs or video footage of me taken during classes or events to be used by the Association for promotional or educational purposes, unless I specifically request otherwise in writing.

8. Termination of Membership/Training:

I acknowledge that the Association reserves the right to cancel any membership or remove me from any training event at its discretion. In such cases, a refund may be issued for the unused portion of the membership/training event fee paid. Refunds are solely at the discretion of the event organiser.

9. Declaration of Claims:

I declare whether I am currently involved in any workers’ compensation claim or litigation for personal injury. I understand that I am required to disclose this to the Association, and failure to do so may affect my participation and the Association’s liability.

10. Governing Law:

This agreement is governed by the laws of Western Australia. If any part of this

agreement is found to be invalid or unenforceable, the remainder shall remain in full force and effect.

I provide the emergency contact information below and consent to the Associa-

tion contacting this person in the event of an emergency. This information will

be handled in accordance with applicable privacy laws.
